CONSENT TO PERSONAL DATA PROCESSING
I hereby expressly consent to the processing of the following personal data by OM Real Estate Fund SICAV, a.s.,
registration number 10660844, having its registered office at Sokolovská 668/136d, 186 00
Praha 8, incorporated by entry in the Companies Register maintained by the Municipal Court in
Prague under number B 26154 (hereinafter referred to as the “Administrator”) within the
meaning of Act No 101/2000 on the protection of personal data: my first name and surname,
company name, email address, and telephone number (hereinafter referred to as the
“Data”). This data needs to be processed in order to contact the customer. The Administrator
will process this data for three years. I give my explicit consent to the aforementioned
processing.
[1] I may withdraw my consent to the processing of the Data at any time. If I withdraw my
consent, the Administrator will be entitled to process only basic data identifying me insofar as
is strictly necessary for it to comply with its obligations under the contractual relationship, if
any, between us, and for as long as is strictly necessary to protect its legitimate interests.
[2] Further, I am aware that I may request the above from the Controller by serving written notice
of withdrawal on the Controller at Sokolovská 668/136d, 186 00 Praha 8.
[3] I understand that, under the Personal Data Protection Act, I have the right to withdraw my
consent at any time, to ask the Administrator for information on the personal data that is
being processed, to ask the Administrator for explanations regarding the processing of the
personal data, to demand access to this data and to have it updated or corrected, and to
request the Administrator to erase this personal data, and that, if I have doubts about
compliance with obligations related to the processing of personal data, I may contact the
Office for Personal Data Protection at Pplk. Sochora 27, 170 00 Praha 7.
I expressly declare that I am entitled to give this consent, that I am over 18 years of age and that, by
providing the Data and other information to the Administrator, I have not breached any legislation of
general application. I further confirm that the Data and other information I have provided to the
Administrator about me is true.
